Identifying Potential Immunological Risks

 

Skin Sensitization Testing evaluates the potential for a medical device's chemical constituents to cause a delayed-type hypersensitivity reaction (Type IV allergy) after repeated or prolonged exposure. Unlike irritation, which is an immediate localized response, sensitization involves the immune system and typically manifests after subsequent contacts.
 

1. Key Objectives

 
  • Immunological Evaluation: To detect whether leachable substances (such as residual monomers, plasticizers, or metal ions) can trigger an allergic response.

  • Clinical Safety: To ensure the device does not cause allergic contact dermatitis (ACD) in patients during its intended use.
     

2. Standard Test Methodologies

 

Under ISO 10993-10, three primary methods are recognized:

  • Guinea Pig Maximization Test (GPMT): The most sensitive standard test, utilizing intradermal injections and topical applications. It is preferred for devices with internal tissue contact.

  • Buehler Test: A topical-only application method, often used for devices that only contact intact skin.

  • Local Lymph Node Assay (LLNA): A quantitative mouse model that measures lymphocyte proliferation. It is considered a more refined method (3Rs) for assessing sensitization potency.
     

3. Overview of the Test Procedure

 

The study generally consists of two phases:

  1. Induction Phase: The animal is repeatedly exposed to the test substance to allow the immune system to become sensitized to any potential allergens.

  2. Challenge Phase: After a rest period, the animal is exposed to the test substance again. The site is then monitored for signs of an allergic reaction, such as erythema (redness) and edema (swelling).
